Ordering Information

For sample requirements for specific tests, see the Test Profiles section and refer to the following general guidelines:

  • Samples shipped to INBC must contain the patient/donor name, one identification number (SSN, DOB, Hospital Number, or UNOS number), and the sample draw date.
  • Blood samples must be shipped at ambient temperature
  • Samples shipped to INBC must be sent with an HLA Request Form.
